Place the tortilla on a clean, dry grill. Sprinkle the cheese, chopped chiles and tomato over the entire tortilla, then top with the hot taco filling.
Fold the tortilla in half and cook until the tortilla begins to brown. Turn over and cook until the cheese is completely melted and the tortilla begins to brown.
Remove from grill and cut into 4 uniform wedges. Garnish with the lettuce and sour cream. Serve with salsa.
